Community Team

Addiction Counsellor

We have a weekly clinic, working closely with Dr Henrietta Brook, to help patients with problems of alcohol and drugs. They can work to reduce harms from street drugs and alcohol, provide substitute opiates on prescription and can also help people who want to cut down their use of addictive substances.

Clinical Pharmacist

We have a growing team of pharmacists and pharmacy technicians. They help ensure that our patients are prescribed the most appropriate medication for their medical conditions. You might be contacted by them to discuss your medical condition and medication.

Primary Care Plus Worker – Mental Health

We have a weekly clinic run by a member of the local mental health NHS Trust to help people with mental health needs.

Social Prescriber

Social prescribing is a growing movement that connects people with a range of non clinical programmes, services and events in their local community like walking groups, mental health counselling, debt management advice, employability skills, volunteering, family fitness, healthy cooking programmes, mindfulness sessions, and housing issues. Often these issues can be complex and challenging, and GPs may refer you to this member of the team to provide you with more time and support to help identify and help with these challenges.

First Contact Physiotherapists

The First Contact Physiotherapists (FCPs) are advanced practitioners working within primary care with extensive expertise in the clinical assessment, diagnosis and management of musculoskeletal (MSK) conditions (MSK conditions are injuries or pain arising from of the muscles, nerves, tendons, joints, cartilage, and spine).

FCPs see patients with (suspected or diagnosed) MSK conditions as the first point of contact, instead of a GP, and can be accessed directly by contacting the Practice’s reception. This gives the patient direct access to a MSK specialist.

Their role is to work with patients to decide on the most appropriate management of their condition, through education and self-management advice, and where appropriate refer for investigations, a course of physiotherapy or make referrals into secondary care services using the same pathways as GPs.

Health and Wellbeing Coach

Health and Wellbeing Coaches see our patients at the Practice working with them to focus on improving their health through a series of coaching sessions using motivational interviewing to support patients to adopt healthy behaviours.

Attached Staff

We are proud to host a wide range of additional members of the practice team who can provide more help and support to people. This can often complement the support that can be given by our GPs and practice nurses.
