Register as New Patient
Welcome to Lavender Hill Group Practice. We are an NHS family practice, providing general medical services to people living in Battersea and Clapham. You are welcome to register with us if you live in our practice area. This area covers most of SW11, SW8 and SW4 within a mile of the surgery (see map).
We hope that you find the practice helpful and accessible by whatever means you contact us. If you need any assistance with communication please let us know as soon as possible so we can arrange appropriate support when you have appointments. This website is designed according to latus accessibility guidelines, you can read more about this on our accessibility page.
Usually, you can come into the surgery and fill in the registration form. However, at present due to COVID we are asking our patients to do this online if possible. If you are unable to do this online, please do call the surgery and we can arrange for you to collect paper forms for completion at home.
There is some information with which it is important to be familiar before you register as a patient. We will ask you to make a declaration about this on the registration form.
How the Practice Works
Please read the Your Health Record page for information about the data we store, how we use and share it.
You will be able to record your preferences on the registration form.
Please read our Online Services page so that you can make an informed decision regarding registering for this when you complete the registration form.
Feedback and Complaints
We welcome feedback, either about what we are doing well, or in areas where we could improve. Please visit the Reviews, Suggestions & Complaints page to familiarise yourself with how to do so.
We also have a Patient Participation Group which meets a few times a year (virtually at the moment) to discuss matters relating to the practice. You will be able to express interest in joining when you complete the registration form.
Zero Tolerance Policy
Lavender Hill Group Practice has a duty to provide a safe and secure environment for patients, staff and visitors. Violent or abusive behaviour will not be tolerated and decisive action will be taken to protect staff, patients and visitors. Please read our Zero Tolerance Policy. You will be asked to say that you agree with it when you register.
Please complete the correct online form and submit it online. If you would prefer to fill the form in another format please contact the surgery.
You will need to bring in proof of address and photo ID but we are currently delaying the need to see these due to COVID. We will contact you once we are able to process these again.
We aim to put your details onto our system within 2 working days. If you have an urgent medical problem that cannot wait 2 days, please call and let our reception team know so that we can expedite your registration. If you have had a smear test privately or abroad, please bring or send in copies of any documentation so we can update our records.
Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
